Dependency Case Manager Administrative Assistant

JOB SUMMARY

This position provides comprehensive case management administrative support to the case management team. The DCM AA is responsible for tracking, monitoring and reporting specific confidential data for the children served in the program. DCM AA assists with completing intake, admission, and discharge documents. Completes timely reporting in the FSFN, ChildNet portals and other applications in place.

ESSENTIAL JOB FUNCTIONS

RESPONSIBILITIES

  • Provide administrative support to all case management staff for children placed at SOS and through discharge.
  • Completes Client Satisfaction Surveys bi-annually.
  • Completes Exit Interviews with clients discharging from program.
  • Inputs information in ChildNet’s Portal within 24 hours of placements and discharges.
  • Assistance with the ongoing maintenance of the case files.
  • Assists with completing intake interview packets and sets up case files.
  • Enters data in FSFN within 48 hours.
  • Tracks, monitors and records data specific information.
  • Assists with completing Ansell Casey Assessments for children in care.
  • Assists with maintaining the Child Resource Records (CRR)
  • Completes Peer Review Files on Medical and Mentor Files.
  • Track, log and maintain incident reports.
  • Files in case files as needed.
  • Monitor and update all master lists including the department report, case management and foster home house lists.
  • Track, log and maintain incident reporting system.
  • Collect, coordinate, and track all school information.
  • Assist with tracking tutoring data.
  • Assists with completing daycare enrollment.
  • Actively participate in the intake and placement process, providing each client with an orientation on the day of placement and any orientation information on the NFH program.
  • Work in partnership with foster families to help achieve permanency.
  • Gather case information about the child(ren) and family to support decision making by the court and ChildNet’s System of Care.
  • Maintain case file from inception through closure.
  • Perform other duties as assigned

MINIMUM EDUCATION AND EXPERIENCE REQUIREMENTS

  • Bachelor’s Degree in Human Services preferred.
  • Associates Degree or higher
  • Valid Florida state issued driver’s license and insurance required.
Pay:
$35,000 to $40,000/year
